Move install commands, canonical sources, and liquid-glass CSS skeleton out of the main SKILL.md into reference/*. Reduces main instruction file from 1206 to 991 lines (-18%), loading only on demand.

Extracts Appendix A/B/C from SKILL.md into dedicated reference/ files. Main instruction file goes from 1206 to 991 lines.
Appendix A/B content is preserved. Appendix C is reformatted as a standalone CSS reference rather than an inline Markdown appendix.
