toc levels default add only first level to ToC.

default the plugin setting "plugin»dw2pdf»toclevels" is empty. Which means it should use the ToC levels of DokuWiki, default 1 to 3. Now only toplevel seems used.

in ConfigTest there is also a test for the default toclevel values. But the Config() is fed an empty array, instead of the default plugin config values.

At least toclevels entry has to be added for that test. The test considers then the default plugin config value, what means it will fail.
